To get started:
- Review the registration steps below
- Setup a patient account in our system.
- Complete the registration and new patient intake forms.
- Call our office to schedule an appointment.
Steps
Step One: Which Provider Is Best For You
Step Two: New Patient Registration / Patient Portal Setup
- Once you’ve decided that you are ready to schedule an appointment, click here to begin the registration process & create patient portal account within our system.
- Next you will receive a verification email that includes a link allowing you to verify your email and log into your patient portal account.
- After logging into your patient portal, be sure to create a PROFILE PICTURE under the MY ACCOUNT DETAILS TAB. This will make sure we know who to look for during your first appointment.
- *No need to fill in the insurance information since we do not file claims and won’t need it.
Step Three: Intake Forms, Medications & Medical Docs
- Click on QUESTIONNAIRES & complete all of the forms listed under NEW PATIENT.
- **No need to fill out any of the forms listed for established patients and disregard the Medicare forms unless you are on Medicare. Feel free to save your progress and come back later if need be.
- Next, click on MY MEDICATIONS, MY SUPPLEMENTS & MY PHARMACIES to add all of the relevant information currently related to your health.
- Finally, click on LABS & DOCUMENTS and use the BLUE UPLOAD NEW DOCUMENT button to upload any relevant past labs or medical docs from the past 12 months.
